The outer corner of the eye, the lateral canthus, is where the upper and lower lids meet and anchor to the bone through a small tendon. The angle of that corner — tilted up, level, or down — quietly sets much of what people describe as a sharp, gentle or tired impression.
Lateral canthoplasty repositions this anchor point. Lowering the corner is requested by people who feel their upturned eyes look stern; raising it appeals to those whose drooping corners read as sleepy. Both are among the more delicate eyelid procedures, because the corner resists change and small errors are conspicuous.
What the corner angle does to a face
Clinicians describe the line between the inner and outer corners as the canthal tilt. A slight upward tilt is common and often considered youthful, but a steep tilt can make the eyes look narrow and the expression severe, especially when combined with a short horizontal eye length.
A downturned corner has the opposite effect, softening the gaze but tipping toward a fatigued look as it deepens. Corner raising tightens and re-anchors the tendon higher on the bone to lift a drooping outer corner, sometimes alongside procedures that lengthen the eye outward. Both directions share a stubborn opponent — the tissue’s tendency to drift back toward its original position over months, which is why surgeons discuss partial regression as an expected part of the outcome rather than a failure.
Risks, recovery and realistic expectations
The possibility of side effects here is meaningfully higher than in simpler lid procedures: asymmetry between the two corners, over- or under-correction, conjunctival irritation, visible scarring at the corner and lasting regression are all documented outcomes. The recovery period typically runs weeks for swelling, but the final corner position can take several months to declare itself.
Because expression change is the whole point, the consultation matters as much as the operation. A board-certified plastic surgeon should walk through simulated angles, explain how much change is anatomically available in that particular eye, and say plainly when the requested look is not achievable — a smaller change that holds is worth more than a dramatic one that drifts.
