Ptosis — an upper lid that stays low even at full effort — is a function problem, not a shape problem. Yet because most ptosis repair in Korea is performed together with double eyelid surgery, many people with monolids assume that fixing the droop means accepting a crease they never wanted.
That trade is not mandatory. Techniques exist that tighten the levator system while suppressing the internal adhesions that would otherwise print a fold on the skin, preserving the smooth monolid line. The operation demands finer judgment than standard repair, which makes candidate selection and surgeon experience central to the outcome.
Spotting ptosis behind a monolid
On a monolid, drooping is easy to misread as a heavy or thick eyelid. The telltale signs are functional: the lid margin covering part of the pupil, eyebrows that ride high from constant forehead effort, a chin that tilts up to see, and eyes that look markedly smaller in photos taken when the face is relaxed.
Confirmation comes from measurement, not impression. Clinicians check how far the lid margin sits from the center of the pupil and how many millimeters the lid travels between looking down and up with the brow held still — numbers that separate true levator weakness from mere skin hooding, and that show considerable individual variation from person to person.
How the lift works without printing a line
A double eyelid crease forms when the lifting apparatus becomes attached to the skin, so the skin folds each time the eye opens. Fold-free ptosis repair does the opposite of crease surgery: the levator muscle or its aponeurosis is shortened or advanced to raise the margin, while the surgeon avoids creating skin-to-muscle connections and manages the tissue planes so no fixed fold develops.
The approach can run through a small skin opening or from the inner surface of the lid, depending on how much correction is needed and the lid’s thickness. Mild to moderate droop with reasonable muscle function suits the fold-free route better; severe weakness may still push the plan toward conventional repair, a limit worth hearing about honestly in advance.
Recovery, risks and the consultation that matters
Swelling during the recovery period can temporarily crease the lid even in a well-executed fold-free repair, and the smooth line usually returns as tissues settle over weeks. The possibility of side effects includes under- or over-correction, asymmetry between the eyes, dryness from stronger lid opening and, in a minority of cases, a faint line that persists and needs revision.
Because the operation balances two goals that pull against each other — more lift, no fold — the pre-operative conversation should be unusually specific. A board-certified specialist consultation ought to cover measured levator function, the realistic ceiling of fold-free correction for that eyelid, and what the revision plan is if a line appears anyway.
